Concern about penis appearance and performance is one of the primary reasons men seek medical care, for good reason. The penis is an important part of a man’s sexual health, and any changes may be a cause for concern. In adult men, increased curvature of the penis may be associated with Peyronie’s Disease, estimated to affect up to 10% of men over 18, and as many as 23% of men over 40. Interestingly, although the disorder was first described in 1743, the number of cases has increased significantly in recent years.

Over time, those injuries can lead to the formation of plaques of scar tissue that may cause pain during intercourse or indentation on the surface of the penis. However, the most common symptom is increased or abnormal curvature of the erect penis, and is also often associated with loss of length and girth in the erection. Unwanted curvature can make intercourse difficult and painful for a partner, and most men would prefer penile curvature correction. In severe cases, Peyronie’s can cause complex curvature and even bottleneck deformities of the penis.
Peyronie’s Disease may make sexual intercourse painful or difficult, and decrease the pleasure of the partner. Many men suffering from the disease experience depression, erectile dysfunction, and sexual withdrawal. Over 75% of sufferers report stress related to the condition.
Treatment for this condition is complex. In only about 13% of cases, symptoms go away with no treatment. Unfortunately for the rest of the population, there have been few rigorous clinical trials for effective medical intervention.
Oral medications are often prescribed, including vitamin E and potassium amino benzate, a B-complex vitamin. While potassium amino benzate has been shown to reduce plaque size, it does not correct curvature, and involves taking 24 pills a day for 3 to 6 months. Side effects include upset stomach, and it is an expensive treatment.
Prescription anti-inflammation drugs have also been used in the treatment of Peyronie’s Disease, but have not been tested against a placebo, or, when tested, were not proven to be more effective.
Treatment may also include penis injections. A large clinical trial of interferon injections recently demonstrated improvement in curvature, pain, and function, but interferon is expensive and has flu-like side effects. A small study of the effects of injecting the enzyme collagenase was done in the early 90’s, and demonstrated modest improvements, but larger tests have not been completed to date.
In cases of severe but stable Peyronie’s Disease, surgery is the option of last resort, and is reserved for only the most dramatic symptoms. Many surgical techniques correct unwanted curvature, but do so at the cost of some penis length.
Luckily, for most men suffering from Peyronie’s Disease, penis traction devices have shown to be an effective, lasting treatment.
